Technology Requests for New Users, Changes or Additions for Existing Users, and Termination Requests
New User Requests Notice
A notice of 5 BUSINESS DAYS is required for the set-up of a new user to accommodate preparing the account and equipment for the new user.
All new users will be required to take an IT Security Training course and read and acknowledge the Acceptable Use Policy, before access credentials are provided to the Archdiocese Information Systems. Below are the steps in the process for a new user request:
- The ADI (Archdiocese of Indianapolis) manager creates a new user technology request and submits the request to the AME OSC (Operational Support Center).
- AME OSC will send an email containing the username, password, and website to access the IT Security training (provided by KnowBe4) to the ADI manager.
- The ADI Manager will forward the security training information to the new user. The new user can complete the training from any devices that has Internet access. Click on new user IT training process for details of this process.
- New user will complete both the IT Security Training and reads and acknowledges the Archdiocese of Indianapolis Acceptable Use Policy.
- Upon completion of the training, an email will be sent to the ADI manager notifying them that the KnowBe4 training has been completed.
- The ADI Manager will forward the KnowBe4 completion email to the AME OSC to obtain credentials for new user to access the Archdiocese Information Systems. The AME Manager can call the AME OSC to expedite this process after the email has been sent.
- The AME OSC will contact the ADI Manager to provide the credentials to access the Archdiocese Information Systems.

These forms require you to be logged into Office 365 online to access. Please enter your email address (username@archindy.org) and password to login and access the online forms.

This form is to be used when requesting a new user account be created for a user who is ONLY needing Catholic Center door access and WILL NOT be needing computer access.
E.g. Building maintenance, contractor, etc.
